Is your organization changing its name or moving to a new location?
Please complete the following document for each applicable API Registration you currently hold:

ISO 9001 Registration Agreement

API Spec Q1 and ISO/TS 29001 Registration Agreements

ISO 14001 Registration Agreement

If you are moving to a new location, please include the following information with your correspondence:

1. What is the distance from the old facility location to the new facility location?
2. What is the approximate percentage of employees transferring to the new facility?
3. Will the same Quality Management System be used at the new facility?
