Is your organization changing its name or moving to a new location?

Step 1. Download the API Licensing/Registration Renewal Application

Step 2. Download the API Monogram Renewal Package (Please complete one for each API Monogram License you currently hold)

Step 3. Download the Product Specification Form(s) that correspond to the License(s) for which you are currently licensed.

If you are moving to a new location, please include the following information with your correspondence:

What is the distance from the old facility location to the new facility location?
What is the approximate percentage of employees transferring to the new facility?
Will the same Quality Management System be used at the new facility?

Once you have completed the forms, please email them to certification@api.org or fax them to 202-682-8070.
